For this to happen, you need a translation pattern in place. Kindly go through documentation where you can configure this according to your requirement and try.
Stephen, Just a comment , you can easily achieve this by using trusted listed feature in the cube. Instead of no ip address trusted authenticate command in voice register global,define your internal network and also provider network and try this.

Have you try changing the FxO module to see configuration looks fine. Most of the disconnection issues are explained on this .

Seems like you are in stuck .. you can post your Gateway config and also the call routing screen shot from cucm , we shall try what is happening in background.
Please grade out credentials/Ip address.
Seems like trusted list in voice register global making the issue.
Can you remove the this command in voice register global or add those trusted ip and try an outgoing call.
