For #2 look at "SCA Sticky Auto Line Seize" on the Phone tab in the admin+advanced config, turning it on does exactly what you want for incoming calls.

Thanks - this indeed works! Searching backwards through the Admin manual I see mention of 'vid' in the Programmable Softkey section which is obviously not where I was looking.
