Seems there is a bug with update 4.1(2a) for UCS C240 M5SX server. We have upgraded a few servers and PSU is no detected. In the logs some errors:

issue was QoS System Class count, when doing FcoE with Brocade SAN switch it works without C3 discards when you have 2 Classes defined and with more issues start to appear.
