Cisco CP-BEKEM-3PCC key expansion module is occasionaly not recognized

One of my customers has a specific problem, which im unable to solve. Hopefull someone's able to help me out.

 

Problem:

Cisco CP-BEKEM-3PCC (Key Expansion Module) is occasionally not recognized by Cisco CP-8851-3PCC-K9 (with Multiplatform Phone firmware) when you connect it to a PoE + switch.
