SPA 962 doesn't start

SPA 962 doesn't start. when I connect it to the switch - it gets power, but the port doesn't come up, port shows "notconnected" and the phone screen only blinks. Also 4 from 6 line buttons are amber. Can anybody tell what happened with it?

Dear Sir;

This could be either a phone with issues, or issues with the switch power feed. Please ensure switch is ok (test another phone). If ok, then phone has to be RMA.
