Hello,
I've had the SPA504G phones for over 5 years. Yesterday my regular list of extensions was gone and only 770101 is showing on the display. It allows outgoing calls still but no one is able to call in via my regular extension.
How do I get my old settings back?

Just configure it the same way you configured them 5 years ago.
Or I missed the issue ?

Unfortunately, no one can just guess correct configuration for particular environment.
Phone is using either SCSP or SIP protocol to communicate. Phone is registered to a server of VoIP operator. Phone is using a credentials to register. One or more extension number needs to be configured. Audio codecs and supplementary services may or may not require specific settings ... You just need to know your operator requirements. Ask your VoIP operator for instructions (it assumes you know who's your VoIP operator - another fact no one can just guess).
OR
VoIP operator supports remote provisioning of SPA504G, You need to know no facts but URL of provisioning server. Even in such case - ask your VoIP operator for help.
