UC540 on 8.6.1 fail to transfer calls on second call tansfer

Hello we upgraded a UC540 from 8.0.5 to 8.61. After the upgrade we started having call transfer issues. A call would come in to the reception, they would transfer the call to another extension i.e (ext 204). If ext 204 tries to transfer to another extention the transfer fails. The outside call hears silence and ext 204 can not resume the call.

We thought it may be due to the upgrade so we replaced the UC and started from scratch on software pack 8.6.1 and we are still experiencing the same thing.We think it may be related to SNR becuase with SNR off things seem to work...

Any thoughts? Cisco tech support has escalated the case and they still can not figure it out?
