Customized Web Authentication Page on WLC5508 not working?

I am setting up a WLC5508 for guest access and trying to create a customized Web Authentication Page to be loaded back on the WLC. However, the "Submit" button on the login page is not working when user clicked on it. Does someone has a working Webauth Bundle that can share with me or advice what could be the problem? Thanks.

The problem could be because you modified the link in the HTML file. You can download custom webauth bundles from CCO. Just look under the wlc5508 downloads.
