AIR-CAP1602E-A-K9 gets ip address but cannot be seen on WLC

AIR-CAP1602E-A-K9 gets ip address but cannot be seen on WLC.

I used debug mac addr, debug capwapp events enable, errors enable. (Please see attachment).

AP is blinking from amber, green and red.

Ping on the AP is ok and then it will timeout for some time, then will be ok again.
