if i use MR42 with Enterprise License and i need to make policy restriction based on users locations do i need any other hardware device or licence ?

You can only get an "Enterprise" licence for MR units.  There is no other licence type available.

I'm not sure what you mean by either "location" (which AP they are attached to?) or what type of "restrictions" you are meaning.

I mean if i have a hospital and i need to block the internet for the doctors in the cafeteria area  .so i mean i need any hardware appliances like ISE to do this feature beside the Meraki CMX solution ?

No you don´t need any other hardware. You can use their Firewall & traffic Shaping feature to solve you wish.
