SSID are not broadcasting? - Any idea how to fix this?

is it a standalone AP or AP with WLC ?

It's lightweight AP managed by WLC.

Paste the output of these commands:

sh sysinfo from WLC

sh version from AP

also reboot the AP and paste the complete boot-up process from console.

It looks that Radio interface is UP!!!! still you cant see SSID ?

then check if there is any AP groups create don WLC ....WLAN > Advanced > AP Group!!

Yes! It is up and I can see the SSIDs. I have 1400 sites and 3500 APs all in one WLC. It happens from time to time that a single location encounter that problem and after a restart of AP the SSID reappear. It started happening after I upgraded the WLC. All 1400 sites are memeber of [(AP Group Name) with spesific group].

What could be a permanent solution?

I would say to create a TAC case and ask for permanent solution.
