SSID disconnection issue with IP Phone and Adroid

We have one issue with WLC 5508 and we are facing a issue with IPhone and Android users connecting to SSID automatically disconnects and connects again to same SSID this happens frequently

 

Opened a case with Cisco TAC Engineer and the suggested to add a new access point  still the same issue 

Any help
