We have found that only Cisco 1231 WAP are exhibiting this behaviour. Their Primary WLAN controller is Cisco1 WLC but they fail to register
to Primary WLC and fall back to Cisco2 WLC. After about 200 sec , they attempt to connect to Primary WLC once again and fail. The whole cycle is repeated every 250 seconds.
Please help to trobleshoot this Issue ASAP.

Make sure your host name for the primary, secondary and or tertiary is correct (case sensitive). Ip address also you should check. Make sure master WLC is disabled and mobility is up between all WLC's. AP fallback of course should be enabled.
You mentioned it's only an issue with the 1231? So do you have a mix of AP's on the same WLC as the 1231 that you have no problem failing over to another WLC? Are you doing priority for high availability at all and I'm assuming you do have adequate license for all the AP's.

Another possible reason :
Some of the 1231 Access Point use SSC (Self SIgned Certificate). Ensure that you do have that SSC hash on both the controllers associated with the MAC. Without it the 1231 will not join.

During the failover you can always do a debug capwap events and you will see why the aps fail to join. Also, are the controllers in the same mobility group ?
