What IOS version is used? Do you have update the IOS? If yes, can you do a rollback?

I had the same issue when i wanted to upgrade from firmware 150-2.EX5 to 152-6.E2.
This is my workaround:
After this procedure some of our switches can boot with the new firmware right away, some other switches needed one reboot if boot stucked again.
I could upgrade all of my switch with this workaround.

Good afternoon I'm with this same problem when updating the 2960x this caught, the led syst is blinking and when accessing this via console it comes as a switch: and I can not do anything, somebody can help me to solve this problem,
as some options that I see already posted I can not do anything because this way I can not enter the privileged mode.

I resolved this issue today by issuing this command:
switch: boot -skip_post
