MWI not working for 1 user

I have an issue where 1 user on my system is not getting MWI when they get a voicemail.  I have tried to delete and re add the mailbox and I have replaced the phone with a new one.  I also ran "Synchronize All MWIs on This Phone System" on the CUC.

 

I am running 12.5 CUC and CUCM.

Do you have the right voicemail profile attached to the DN in CUCM? 

Assuming a SCCP integration between CUCM and Unity, can you dial the MWI off and on DNs from the affected phone? 

I have the same voicemail profile as the other phones at this site.  I was able to turn the MWI on and off by dialing the number however still doesn't work when someone leaves a message.

In Unity, is the affected voicemail account attached to the same Phone System, Partition and Search Scope as other accounts which get MWI notifications? 

Yes there is only one phone system setup in Unity and this device is associated to it.
