Was working fine up until a few weeks ago.

What else have you done to troubleshoot? If you rebooted a switch and then the ap never joined, did someone not save the config prior to the reboot? Have you verified the port configuration and that the ap is getting an ip address? Your best bet is to console into the ap, reboot that ap and look at the output. That will show you where its failing.

1. Make sure the AP has enough power
2. I don't see any attempt to join a controller - what discovery method are you using?
3. You may have hardware failure (AP critical) - in which case RMA
