I have experienced the same bug on the B200 M5 blades when upgrading to firmware 4.0(4)b and the suggested workaround also works there.

If you experience this error on FX-OS based firewall, such as the 2100/4100 series, there is no UCS Manager to reset the controller. 

The commands to reset on an FX-OS Firewall are:

scope server 1/1
scope flexflash-controller 1
reset
commit-buffer
