You need to connect to the AP via console and check the logs there. There are many reason for the AP disassociate from the WLC. Check the connectivity, make sure the AP is up, you can see it on the CDP from the switch it is connected.
the AP is connected, from the controller I can ping this AP also from the AP I can ping the controller
I typed the command show cdp neighbors I saw who is connected
when i connect via console i get the errors as shown attached
