Understand, while the two sites mentioned are now properly identified and subject to users' filter settings, there are very likely other sites that were similarly awaiting votes.

It has been my experience with the MyFi predecessor, LaptopConnect, that AT&T won't let you use an alternate DNS like OpenDNS. I haven't tried it on a MyFi yet.
Afraid not; there's no way for OpenDNS to see the origination IP of the request (and thankfully so). You will need to determine whether your router has such logging capability, or a third-party application that can do it.
