are you running an 11ac client and AP by any chance.
MCS9 2SS 11ac gives you above 300Mbps.
MCS9 2SS 11n only gives you the mentioned 54 or 60Mbps

The WLC uses the first DHCP message from a client to build a client list for mobility. Then drops the first packet. When the client sends the second DHCP discover it gets forwarded.
But this causes a delay in DHCP assignment....
no expert on the coverage hole detection algorithm.
But if there is a client issue, I would assume this happens at multiple locations (assuming more than one client - and the clients do CCX). Mutliple locations would mean more APs.
