And setup a SYSLOG Server and logging to it from this and other devices. Turn up logging as high as possible at until you find the issue.ALSO What modules do you have in this chassis.

On second thought it looks like someone has configured a password. If you are getting this message through a console cable then PW recover is the option.AND Im not sure if the cna works with 4500's
