My CE 560 is constantly failing to cache objects because of this missed reason

object_in_cache_older_than_clients: 14

What does it mean? I've tried clearing the cache and the problem still persists.

The content is already in the cache, the CE just isn't serving it (because 'object_in_cache_older_than_clients'). This would have to be because the HTTP If-Modified-Since (IMS) request from the client has a date greater than the date associated with the content in the cache.
