CSCue28788 - All ports went into "Hardware failure" state on FI after Auto install

Hey Guys,

 

Has anyone heard of this?  We recently updated the code on one of our Fabric Interconnects and when the Interconnect came back up, the first 8 ports were in a failure state.  I am concerned about future upgrades.  Thoughts?
