Guest Hotspot "custom Fields" ?

Hi

My Customer is looking for a hotspot type experience (no registering) but having the User input some info fields ( Name phone number etc) before clicking accept to connect ..

Customer doesn't like the normal self register experience , and Says  hotspot doesnt allow them to "Collect" some User data .

I cant seem to figure out if this is possible with ISE ?

Ideas ?
