If we have a "L-ISE-BSE-5K=" license for ISE, how many VMs am I allowed to create to host the various roles?

That license allows you to authenticate 5K users and some other basic features. You need another license to entitle you to own the ISE software to build virtual appliances, but there is no actual enforcement of it.
The license is ISE-VM-K9=. You need one per appliance. There are also bundled SKUs.

The number of appliances and the size of those appliances depends on how many endpoints you are going to have in your environment. If you are only going to have 5K endpoints or less total, you could get by with two ISE nodes sized as a 3415 appliance.

Thank you, but if I want to plan for growth and I have the resources I could use 3495? or is it dependent on what VM package we purchased?

If you have the resources, then go ahead and use the 3495 appliance sizing specifications. Scaling ISE is very easy to do. You can add more nodes to any ISE deployment as you need and you can move roles (admin, monitor, policy services) around from node to node as you grow.
The ISE-VM-K9 license (1 per virtual appliance) simply entitles you to install the software and use whatever sizing specifications you want.
