Tacacs User Password Reset

I have read about article there is on creating a "my device portal" for User password resets and I have not been able to resolve the issue of when a user logs in, it just brings them to a blank screen. 

 

I have entered the scripts in optional content 2 box with no luck. 

 

Can someone give me a hint on where to look for what is wrong?

It could possibly be a bug with your browser or the WebUI in ISE. I suggest resetting the cache in your browser as a first step. If that doesn't resolve the issue, then please contact the TAC.
