CSCvc07112 - has happened 4 times to just one 2130 pair - why? How to fix it?

Hi,

I have 4 x pairs of 2130 with about 18-22 contexts max on them.  One pair has consistently hit this bug and there seems to be no way to determine why/how?  

anyone have had this and what was the cause.
