Can Stateful Failover be done WITHOUT the serial failover cable on a PIX with version 6.0? If so how?
If not, has anyone extended the cable using a modem, CSU/DSU, or line driver? If so how please be specific (Products used and configuration used)?

I've only used failover with the serial cable.

It is mandatory to use FO serial cable in both stateful and normal failover. Serial cable is mainly used to assign roles to each unit (primary or secondary) and to Synchronized bith pix configs.
Cross Over cable, in statefull is mainly used to exchange stratus information such as sessions
etc..
I don´t think CSU/CDU or modem connection will work with serial cable since this one is not a standard RS-232 cable.
