Where can I find a download of the Qualys connector for SF 4000 and relevant documentation?

Any help is appreciated!

Those downloads are still on the old Sourcefire site:

     https://support.sourcefire.com/sections/4/sub_sections/23

I believe using that feature requires activating a license.

The Qualys Connector has a PID.  We sell it.  There is no enforceable license though.
