Symptoms
Factory reset the switch (running IOS 16.06.05) to default settings.
Solution
Power cycle the switch (make sure console cable is connected). Press the Mode button multiple times (that's what worked for me) until the console shows this:
Switch:
Add the following variables to ignore the startup configuration.
Switch: SWITCH_IGNORE_STARTUP_CFG=1
Reset the switch.
Switch: reset
Wait the switch to boot up, and all the settings are back to start.
