I talked with one of the developers here and found out why your 4215 was running 4.0(1)S37 and why it was showing up as unkown.
The re-imaging process first loads the 4.0(1)S37 software (which doesn't know about the 4215) and then loads the 4.1(1)S47 software.
The re-imaging process must have encountered an error or the sensor was powered off and back on before the 4.1(1)S47 software could be loaded.
This left the image in a corrupt (only partially installed) state.
I had never seen this happen before and did not realize that 4.0(1)S37 was first loaded before 4.1(1)S47 during the re-image process.
