As the switch is not even booting to the normal IOS mode without throwing that "software forced crash" message, I would suggest to use a different IOS file in that Flash. If it's an external compact flash, then you can copy the file through a PC with card reader or you'd have to use the xmodem procedure:

As per the log, the reason for crash shows as "Software forced crash", this could be either IOS related or memory problem.
I would suggest you to open a TAC case (if possible, if this switch is under Contract) and provide the following outputs:
- sh tech
- sh file system
- Crashinfo file if available in the bootdisk or flash
If opening a TAC case is not possible then refer to the following link for more details about this type of crash:

You can also think of upgrading the IOS (which is called as Blind upgrade) if this device is not in production and an IOS upgrade can be tested
