Cisco SG200-26 Keeps Losing Setup - How?

I'm using SG200-26. I updated the admin password, then I set a static IP. All good. *BUT* everytime the device is restarted/power down/etc, it LOSES all the setup and goes back to factory default. Not a clue WHY. Never seen anything like it. Can...
