SIP Domain Name?

IS there a way to specify a SIP domain name on the authentication for a SIP trunk? Our provider requires this to be present.. The old admin guide says it was there where has it gone?

Is there anyway to get MOH playing as soon as a user selects an option from the auto attendant so they get music instead of hearing the ringing?

SIP domain doesnt resolve to an IP.. If it use it as a proxy it will fail why has the SIP domain gone anyway?
