I have opened a case with TAC and referenced this community post with them to show that I am not the only one with this issue. For reference in the TAC case, I am running on a 4500X-16 switch, but this appears to be across multiple platforms also.
I got it working again.What I did was soon after I insert the blade before it goes to power down mode I did ahw-module module 8 reset cf:1 After that I could get into maintenance mode and reimage with 5.0(1) again.IDSM-2 is working again for now.
