Yes, our gateway is exposed to public internet. and we have two interfaces with public IP Address configured. Our firewall is at the back of call manager. My network scenerio looks like this:

Yes, you are right. I got a bunch of different IP address today. I couldn't upgrade the system firmware because of circumstances.
