Please check hardware is the same as well as software on both sides. Can you send output of sh access-lists 115 command. It appears that you are hitting bug id CSCdz29226-output ACL applied in ingress E2 is broken. You may hit CSCdx94390 which is a memory fragmentation bug which is the reason for the upgrade to 12.0(21)S5.
