Hello,
It seems that we are facing this issue (CSCvu12664) also on a the following version - 16.12.4 (C9500-48Y4C)
Is it confirmed that 16.12.5 is fixing the problem ?

There are many bugs fixed on 17. X code, so if you are upgrading, upgrade to 17.6.4

according to known fix release in bug you share the 16.12.5 is fix the issue.
