I came across a very similar problem last night. SFP part number FTLF8519P3BNL-C4.
MDS 9250i version 6.2(13b)
When looking at the transceiver though it reports 'digital diagnostics feature not supported in SFP' so it doesn't report as a DOM problem.
The IP storage port just sticks at initializing and when you look at the brief output it shows as initializing 10G.
On the interface we have 'no switchport auto-negotiate' and hence the device it was plugged into (NEXUS) was configured the same.
The only way we could bring up the port was to turn on 'auto negotiation' on the NEXUS port. It looks like the SFP/port requires the autonegotiation to set the port speed to 1Gbps, even though the IPS port is set to 1000 and told not to negotiate.
I would be interested if this resolves your problem.
