After fighting this for months, where a reload may temporarily solve the problem but it always comes back ans destroys the stack... Found that the problem was actually the stack cables - Highly suggest swapping them before doing extensive further troubleshooting if the issue comes back after reload.
If TAC can provide a reliable port mapping - I am guessing that " dest slots:0x8" tells you which is the bad cable.
