Faced power supply issue during 4500 series switch upgrade

We recently upgraded 11 number of 4500 series switches to Cisco recommended version and among 11 we faced issues for 2 switches where Power Supply 2 status was showing n.a

 

Both the switch redundant power supply problem is resolved by Power supply module hard resetting. This problem we have noticed twice after software upgrade was done hence, I just want to know if the software version upgrade is causing this or any other problem we see.

 

Please assist.

Sounds like an IOS bug.  What version did you upgrade to?

Can you explian what is the verison of IOS before upgrade, and can you also post show version of current to give some indication what is the bug reported any

 

have you read the release notes before upgrade ? (always suggested, there may be some information provided expected this kind of news)
