We are getting this error
LDAP: query BOE_LDAP.accept result LDAP server misconfigured or unreachable
Does anyone know what might be causing it.

Have you gone to System Administration/LDAP, clicked on the server profile and then tested the Accept query?
That will often tell you what's broken...(server down? user locked out? password changed? firewall on LDAP server enabled?)...

Check if the appliance has LDAP debug logs created. If not, please do create that.

You can use ldaptest command via CLI to test the configuration of the LDAP profile.
A simple TELNET from the appliance to the LDAP server (hostname or IP address, depending on how it is configured) will work to discard firewall as an issue.
