Okay, I'm scratching this up to the case of the "Mondays" since I can't seem to find the answer, but which email pipeline (ESA) process is tagging the following header to the email?
Header in "head scratching mode" --> "X-MGA-submission"

Hi Chet!
Did some digging as that was a new one on me too!
The "X-MGA-submission" is related to the "Email Submission and Tracking Portal Registration", which is a web-based tool that allows email administrators to report incorrectly classified messages to Cisco and to track the email submissions from their organization. And this header is added only when the registration ID is set in this section.
To learn more about the Email Submission and Tracking Portal Registration, please check chapter 13, "Reporting Incorrectly Classified Messages to Cisco" section from the user guide: http://www.cisco.com/c/en/us/support/security/email-security-appliance/products-release-notes-list.html
