CiscoSecure ACS V3.3 Failed Attempt message

CiscoSecure ACS V3.3 Failed Attempt message "Bad request from NAS". I was able to log into the device successfully. I have verified TACACS key on both CiscoSecure ACS server and on the router to make sure that passwords are the same. This same failed attempt message keep registering on ACS report every minute even when there is no one log into the device. Any recommendations would be greatly appreciated.

When you logged into the device did it authenticate with the ACS or did it use a backup method to authenticate? (one way to know would be to look in the ACS reports for the successful authentication.)

It might be helpful if you would post the config of the router (changing passwords etc).

It might also be helpful if you could post the output of debug tacacs packet.
