IBNS problem logging in

I'm trying to configure IBNS with ACS5.1 using a 3550 as a test switch.

Everything seems to work fine on the outside with the role-mapping, dACL and VLAN assignment, but I let my coworker try to login on my PC after I logged out and got an error saying that there's no logon server to service the request. I figure that the switch is blocking traffic to the domain controller. Right now, the PC is logging me in with cached credentials then I authenticate once explorer starts.

The IP phone also never registers until the PC user authenticates even though MAB is configured on both ACS and the switch (it worked once but I'm not even seeing any logs or matched rules in ACS since then...and I haven't changed anything...I think =/).

No idea how to fix this...any direction would be appreciated greatly!
