Have all IOS commands been assigned either priv 1 or 15 by default since time immemorial? Or at some point in the past were some commands assigned priv level 0 by default?

You answered it. I found a passage in a Cisco Press book that stated that all commands are either priv level 1 or 15 by default, which conflicted with my understanding. So perhaps the book doesn't consider disable, enable exit, help and logout as "commands", or it's a mis-statement. Cisco press is pretty bad about making incorrect statements.
