How can I get uptime report of any device configured in LMS 2.5 ???

I have configured so many devices in the CiscoWorks 2.5 But at the end of day/Week I want to get a report that will show how many times device was go down for this time period ??

Thats not a trackable field in 2.5, or 2.5.1 or even 2.6. It is however available in LMS 3.0.

Is there any way through that I can know that how many time my device (Router & switch) was not reachable OR down ???

You could look at your events/alarms configured in DFM to get an idea of the problems that your device had in LMS 2.5/2.6
