Im VERY MUCH a rookie when it comes to these devices, but Im trying! :)
I had a pair of 4.0.5 CAM's running in HA (non production lab) but was having issues with failover. I decided to download the 4.1 CD ISO and burn a CD to try and upgrade the OS. That all went well but when I boot from that cd, it comes up with the install screen and I cant do anything!
If Im on console (keyboard, mouse, monitor) I hit return at boot: and it goes through the load, until I get to the colorful screen where I enter what I want to do. However at that point I have no keyboard or mouse control.
If I type serial in that space and hyperterm in, pretty much the same thing.
I have tried USB keyboard/mouse and PS2 keyboard mouse with the same results.
Any tips appreciated!

I found that on the CAM appliance which is a DL140 G2, it works fine, but on the CAS, which is a DL140 G3, it doesn't work.
I noticed that when the Linux setup CD was loading the kernel, it says "no ps2 device found", so I imagine that there is a problem with a newer device in the G3 that's not part of the kernel.
I have opened a tac case.

Interesting. The DL140 did work on my CAS, but Ill have to double check the exact model of the box. Thank you for that info, and I look forward to hear what the TAC said in your case.

The NAC 3140 was the G2, and the 3310 was the G3. I purchased a CAM failover bundle light, which was 2 of the 3140 G2's, and one CAS server which was a 3310 G3.

According to the link, you need enter "DL140" at boot. It worked for me.
