I'm doing Video conferencing thru ADSL connection using PIX Firewall 6.3 and doing PAT.Do I need to open any ports or by default it already open by the standard fixup port?

Inbound or outbound through the Pix? If it's outbound, the default fixups for h223 will probably suffice and additional configuration will probably not be necessary. (assuming NAT and routing have already been taken care of for outbound accesss in general.)
If it's inbound, you'll need [static] entries and an ACL to allow the appropriate traffic in to your endpoint.
