40 Gig troubleshooting 2 channels 1 side don't show light

Folks,
I have a 2 Cisco switches with 40 Gig optics in each.  One side sees light on all 4 channels and everything looks good.  The other side does not see light on 2 of the 4 channels.  Where to start troubleshooting? I have tried different ports on the switches.

what the length of cable you use ??
if the length is two short

This is a dark fiber path. It is a 10-mile path.  This is why I am using the ER optics

  Rx Power      -3.94 dBm ++  <<- this make think that the length is short, the recommend length is 40 km and you use 16 km

I am thinking I am going to try an attenuator.  Maybe too hot?

fiber is fine. Can run a 10 gig optic over and has been verified with an OTDR
